Waiver of Liability - Hold Harmless

By attending or participating in Elisabeth Best LLC workshops, retreats, trainings, or in-person physical activities, the undersigned (“Participant”) understands and agrees that participation in either or both of these classes is conditioned upon Participant’s agreement to the terms and conditions of this Waiver of Liability – Hold Harmless (this “Agreement”).

Participant is of sound mind and judgment and understands and agrees to be bound by the terms of this Agreement.

Participant states and agrees, “I am aware of the COVID-19 pandemic and related governmental orders, directives and guidelines (hereafter referred to as directives), including directives for frequent hand washing, social distance, use of masks / face coverings in public locations. I am aware that the activities of this event are occurring in public locations and / or in groups and that other potentially hazardous activities may be involved. I am aware that I could be infected, seriously injured or even die due to COVID-19 or the activities of / associated with this event. I am voluntarily participating in these activities with knowledge of the danger involved and agree to assume any and all risks of bodily injury, death, property damage, and harm, whether those risks are known or unknown.”

Participant further agrees that participation in this in-person event requires proof of full vaccination or negative COVID-19 test taken within 48 hours of event start. When referencing “fully vaccinated” in this agreement, we are following the CDC guidance of at least 2 weeks after you have received the second dose of a 2-dose series vaccine, or at least 2 weeks after you have received a single-dose vaccine.

Participant hereby assumes full responsibility for the risk of bodily injury, death or property damage arising out of his/her participation in or involvement in the above referenced classes, and completely waives, to the greatest extent permitted by law, any and all claims that he/she may hereafter have against Elisabeth Best LLC, and each of its respective members, officers, directors, agents and affiliates (collectively “Indemnified Parties”), which claims relate in any way to, or which arise directly or indirectly, in whole or in part, from the conduction of the events, including claims that may arise out of the negligence or gross negligence of such persons or entities.

Participant further acknowledges that each event requires physical participation and carries with it the potential of serious injury, and property loss. The risks include, but are not limited to those caused by terrain, temperature, weather, condition of participants, lack of hydration, and actions of others in this activity. Participant realizes that liability may arise from negligence or carelessness on the part of the persons or entities listed above; from dangerous or defective equipment or property owned, maintained or controlled by them; or conditions beyond the Indemnified Parties' control.

Participant acknowledges full responsibility and ownership for all decisions pertaining to substances used, ingested, and / or any and all altered states of consciousness entered into during the course of any workshop, retreat, training or in-person event and fully indemnifies and releases liability from Elisabeth Best LLC, our trainers, collaborators, successors, and assigns. Participant attests to the fact that she/they is physically fit and has not been advised otherwise by a qualified medical person.

I provide permission for photographs to be taken and posted on Elisabeth Best's social media, website, and future marketing communications.

I hereby certify that I have read this document in its entirety and all accompanying rules and safety recommendations and understand their contents.

I certify that I am fully vaccinated and will provide proof of vaccination to Elisabeth Best LLC.

I agree to the charges listed above and understand the risk involved in participating in this activity.
